A Nutritional Assessment Questionnaire (NAQ) is a comprehensive tool used by nutritional therapy practitioners to evaluate a person's health status, symptoms, and potential foundational imbalances. It consists of a series of questions that assess various body systems, dietary habits, lifestyle factors, and symptoms related to nutrient deficiencies or dysfunctions. After completing the 320+ question form, we get this calculated graph that helps us to see a clear picture of what imbalances need some support for optimal health.

This first graph indicates where she started as a 6-month Package client. What you can identify is that there are foundational markers that are elevated and out of balance. Her main concerns were diabetes, lack of weight loss (even when being “perfect” with workouts and diet), and wanting to be a good role model in nutrition for her two boys. She had an “all or nothing” mindset when it came to sweets and treats, and when they were taken away, her body craved them. She also noticed so much bloating with meals, even with something simple like a salad, and also noticed daily constipation, leading to days without a movement.

We incorporated some simple lifestyle habits to improve digestion, balance her blood sugar, and help with an afternoon slump in energy. Even though this was going to take time, she agreed to implement and make small adjustments along the way with her HTMA testing and monthly follow-up visits. Over 6 months, we were able to see some amazing results! The graph below is her second NAQ analysis we recently reviewed on her Follow-Up Visit. She shared that the habits + nutrition changes that were implemented also helped her mood, sleep cycles, afternoon crashes & sugar cravings, and diminished her bloating! She’s also having daily bowel movements, has the energy to move her body again, and even got her husband on board with this care, too!
Over the 6 months that we worked together, she has taken small and steady steps towards healing through nutrition and lifestyle! While all of my clients are unique and come with VERY different starting places, there is space for EVERYONE here to heal from the inside out using my services.
